Form 216 | Operating Income Statement

This form provides the lender with the net operating costs of the tenant-occupied property, such as costs of utilities, etc. Information required on this form is provided by the owner/property manager.

This form is optional (depending upon lender guidelines) and is used in conjunction with the URAR. The data requested on this form is often given verbally to the lender or provided at the time of application.
